Responsibilities include coordinate and oversee patient care plans in collaboration with medical staff, patients, and families; perform assessments to identify patient needs, including social, emotional, and financial support; facilitate discharge planning and ensure continuity of care by coordinating post-discharge services; act as a liaison between patients, families, and the healthcare team to address concerns and resolve barriers to care.
