Job Description:
A leading acute care hospital located in southeastern New York
is now searching for an experienced RN Nurse Navigator Orthopedics
to join their award-winning healthcare team.
This acute care facility was founded over one hundred years ago and
today houses nearly 400 licensed beds and offers a wide variety of
medical and surgical services. This state-of-the-art medical center
provides a number of patient care services including behavioral
health, oncology services, cardiopulmonary and vascular services,
emergency and critical care, radiology services, maternity and
pediatric care, wound and home care, and much more.
Reporting to the Nurse Director, the RN Nurse Navigator Orthopedics
is responsible for ensuring seamless transition of care from
preoperative to postoperative care phases. Primary responsibilities
educating patients and families, offering emotional support and
clinical knowledge, provides care coordination across multiple care
provider teams, traverses insurance issues, monitors treatment
progress, serves as a point person for the patient, and ensures
optimal patient outcomes.i
What's Needed to Succeed:
RN Registered Nurse license in the state of New York or ability to obtain
Clinical knowledge of orthopedic care services
Prior working experience in care coordination or similar setting
